Employer Wellness Plans

Partner with Circuit Wellness to establish a healthier employee base and lower health care costs.

Option A)  Employee benefit plan

• Receive a discounted membership rate of $29/mo (regular price is $39/mo) for each employee committing to improve his/her health and wellness at Circuit Wellness.  Monthly membership dues paid by employer 1st of each month based upon # of active employees.  Employer controls list of “active” employees.
  • Complimentary physical activity readiness questionnaire
  • Complimentary nutritional assessment
  • Receive records of workout frequency (if desired)
    • Consider employer sponsored contest around frequency of workouts or % goal achievement
• Opportunity to host company off site (team building) event at Circuit Wellness at no cost
  • Experience “Exergames”
• 20% discount on personal trainer services
• 20% discount on nutritional consults by a registered dietitian
• Complimentary employer advertising through Circuit Wellness member check-in system

Option B)  Pay for performance plan

• Reimburse employees for membership dues ($39/mo) based upon work out frequency goal achievement.  For example, employer could establish a minimum threshold of 8 workouts per month to qualify for reimbursement for that months dues
• Employer can determine what % of the dues will be reimbursed
• Circuit Wellness will provide a monthly report listing the number of work outs by each employee (based upon key card swipe upon entry).  Employee is then responsible for seeking reimbursement through employer established process
• 10% discount on personal trainer services
• 10% discount on nutritional consults by a registered dietitian
• Discounted advertising opportunity through Circuit Wellness member check-in system

Option C)  Partnership promotion plan

• Employer agrees to promote the importance of fitness in partnership with Circuit Wellness to its employees
• Employer distributes promotional brochures to its employees
• Circuit Wellness will provide monthly report on employees work out frequency to facilitate employer initiated wellness incentive
• 10% membership discount offered to the employees
• 10% discount on personal trainer services
• 10% discount on nutritional consults by a registered dietitian
